Vendor Request Form

8’ x 8’ space - $60.00 per space

A 6’ table is provided with an 8’ x 8’space if requested. Additional tables can be obtained for $20/table.

Two badges are provided per vendor. Additional badges may be purchased prior to the convention for $40.00

You must provide your own extension cords and/or power strips.

If you have not requested an electrical connection with your application, the layout may preclude availability.

If no badge names are provided on this form, the name of the company will be used.

Information on this form may be amended until March 23, 2015.

If you have a PayPal account, fill out the above and we will send an invoice directly to you upon acceptance of your application.

Vendor spaces are NOT confirmed until the Vendor’s Liaison issues a confirmation and they have been paid for.

Your application will initially be reviewed for acceptance. You will be informed of your acceptance or placement on the waiting list. If you are accepted you will be informed where to send payment. If the payment is not received by Feb 1,2015, your vendor space will be transferred to a vendor on the waiting list.
